Human in the loop

Also called: HITL

Human in the loop means no AI agent's output, a scored lead, a drafted email, a field written back to the CRM, reaches a prospect or a system of record without a person checking it first, so one gate catches both a hostile prompt injection and an ordinary model mistake.

The gate is the whole mechanism. It doesn't matter whether a bad output came from a poisoned webpage or a model having an off day; a person checking before anything reaches a prospect or a system of record catches both the same way. That single rule does more work than trying to diagnose which failure mode produced a wrong result.

Ownership beats vague oversight

A working human-in-the-loop setup names a specific person for each agent, not a team, who reviews its output on a set cadence. There's a written trigger for when that person has to step in: a confidence score dropping below a threshold, a reply that doesn't classify cleanly, several consecutive skips on records that look similar. When the trigger fires, the agent stops and a person decides, instead of the agent guessing and moving on. "The team reviews it" isn't the same commitment as a named owner with a cadence.

Confidence labeling is what gives the human something specific to check rather than a wall of undifferentiated output. Claims an agent surfaces get tagged verified, inferred, or unverifiable. Only verified claims move into a sequence unreviewed. Inferred claims go to a review queue. Unverifiable claims get dropped or routed to a person to research properly, not softened into a hedge and sent anyway.

In practice

How tight the loop needs to be depends on blast radius, not on how sophisticated the agent is. Scoring or prioritizing inbound signal is a bounded decision: get it wrong and a rep looks at a lead that should have ranked differently, which is annoying, not damaging. Letting an agent autonomously reply to a live prospect or book a meeting is unbounded: get it wrong and the mistake sits in a real inbox attached to a real company, with no quiet way to re-sort it away.

What people get wrong

Human in the loop gets treated as a one-time approval to clear before launch rather than a standing review cadence with a named owner and a rollback path. The other common gap is assuming a diffuse "the team keeps an eye on it" counts as oversight. It doesn't function as a gate unless one specific person is accountable for checking output and empowered to stop the agent when the trigger fires.
